COMPREHENSIVE MEDICATION MANAGEMENT (CMM)
Comprehensive medication management is a patient-centered, health care team (physician, a clinical pharmacist, and patient) approach to optimizing medication use and improving health care. It is a whole person approach that counts in more than just a medication list. This process ensures each patient's medications (including prescriptions) are individually assessed to determine appropriateness, efficacy, and safety. The clinical pharmacist can communicate with the patient's doctor all recommended medication changes to create an optimal medication plan.
PHARMACOGENOMICS (PGX) TESTING
DNA sample collection is an easy and simple process.
We collect a DNA sample by painless swabbing the inside of your cheek or you can collect the sample at home using a self collection kit. For those who prefer self collection, follow all instructions carefully to make sure that proper DNA sample is obtained and avoid the need to repeat the test. Once you complete the collection, you mail the sample following the instructions and using the enclosed prepaid return package. The result should be available within 7-10 days.
MEDICATION EDUCATION AND COUNSELING
The patient meets with the clinical pharmacist, and they discuss the patient's medication history, preferences, lifestyle and goals. All medications are reviewed to ensure they are the best for the patient, and the patient is able to take the medication as intended and adhere to the prescribed regimen. The clinical pharmacist will educate the patient about each medication on what each medication is for, how to take it, and when to take it.
DRUG COST SAVING CHECK
We assess the possible areas patients can save money on their medications. We look for a more affordable alternative medications and any available coupons for brand name medications. We assess any medications that are unnecessary due to duplication, causing drug-drug interaction, or forgotten to be discontinued, then make recommendation to their doctors.

The Comprehensive Medication Consultation fee includes:
*Two appointments (initial and final).
*Cost of the pharmacogenomic test.
*Time spent researching to interpret the lab results in order to make specific recommendations.
*A concise recommendation report will be provided to you after the second visit.
*A card of your gene profile will be provided to you so you can take it to doctor's visits.
Consultation can be done in person or online/video.

Initial 30 min visit- $100.
Final 45 min visit- $125.
Additional Visits
$100/visit
Each visit will be up to 30 minute long.
You can request a visit when your drug therapy changes, new medications are added, or want us to help you manage diabetes, or other chronic diseases.
